Are these maximum data rates correct?
- BPSK -> 6Mbit/s
- QPSK -> 12Mbit/s
- 16QAM -> 24Mbit/s

What is the sampling rate? 10M samples/s or 12,5M samples/s?

The sampling rate is 40MHz. The effective bandwidth is 10MHz (the OFDM ref design interpolates/decimates its I/Q streams by 4). This bandwidth results in the datarates you listed. See this post for more details.
